When I get bored I come up with video game difficulties in my head how about this:
Hardcore.
(Permadeath, all elite mobs have randomized prefix and affixes on top of base abilities. Mob respawn rates increased(to prevent cheese and stalling). AoE silence, disable, slows disables. Hard CC break randomizer increased. More aggressive A.I. Spell caster mobs CC at an intermediate rate; priest casters dispel shields/hits

Scrap permadeath actually that's shit because the difficulty of the game would be int intrinsic to the mechanics. I only realized after playing EverQuest 1999 that random charm and CC breaks, critical fails and that RNG are what makes the game feel alive and challenging. If you know a polymorph lasts a full minute then fuck it. But if it can break anywhere between 5 and 60 seconds...? ohh yeaaa
